Family dentistry · single location

New patients, without the phone tag.

A concept homepage for a two-chair family practice. Same-week appointments, insurance answers in plain language, and a booking request that takes under a minute.

Concept — not a real client. The form below is a layout demonstration and is not connected.

What we do

Three reasons people call.

Most new patients come for a cleaning and stay for the way the front desk handles the rest. The page should answer the three questions they actually have.

01

Cleanings and exams

Routine care for adults and children, including digital X-rays and a written treatment plan before anything is scheduled.

02

Crowns and repairs

Crowns, bridges, and fillings, with the lab timeline and the cost explained before the first appointment.

03

Emergency visits

A sore tooth, a broken crown, or swelling: call in the morning and the practice holds same-day slots for new and existing patients.

Insurance

Answers before the appointment.

The practice bills most PPO plans and checks benefits before the first visit. If you are uninsured, the cash price is printed, not negotiated at the desk.

Bring your plan name and member ID. The front desk confirms what is covered and what is not, in writing, before treatment starts.

Your first visit

  1. Send the form. Name, phone, and what hurts. The practice calls back the same business day.
  2. We check benefits. You get the covered amount and the out-of-pocket estimate before you come in.
  3. Arrive ten minutes early. Paperwork is already done; the first visit is an exam, X-rays, and a plan with prices.

Appointments

Request a time.

Four fields. The front desk confirms by phone, usually within one business day. If this is a dental emergency, call the office instead of using the form.

This concept page does not collect submissions. The fields and the layout are the deliverable; the form would connect to the practice’s booking inbox.
